It finally clicked when I saw the EOB on my desk next to the Vitality Depot’s ad for the new portable Interferential / TENS / EMS / Microcurrent unit. I was reimbursed nine dollars for fifteen minutes of ultrasound therapy and twelve dollars for electrical stimulation. Knowing that it costs me about 20% of every insurance dollar to collect it, and that I had to pay staff to set up the patients, I realized that I was hardly making any money for in-office stim and ultrasound. MY AHA! MOMENT WAS REALIZING THAT I COULD RENT MY PORTABLE STIM AND ULTRASOUND UNITS FOR HOME USE.
In that instant, I could see how providing home therapy options made great clinical and business sense. Why not rent portable units for electrical stim and ultrasound therapy to the patients for the equivalent amount that I get reimbursed to do in my office? With a few low cost units, I can send my patients home with portable units and instructions, and free up one full time staff member in my office! I am supposed to be transitioning patients to active home care anyway, so the home therapy would be a positive move for my in-network patients. With as few as 12 portable stim and portable ultrasound units (total investment for under $500), I could save nearly $30,000 in salary and still earn the same revenue and still provide the same level of care! Ahhh. What about the in-office patient experience?
What about the need to provide pain relief care to the patient in my office? If they can do their own electrical stim and ultrasound at home, then what should I offer in my practice as a replacement? Laser therapy has the panache of being cutting edge and super-advanced technology. I overheard folks talking at the gym that they should go to a particular pain therapy clinic because they had laser treatment. I realized that although the electrical stim and ultrasound therapies were beneficial, I had never gotten a patient referral for them, but patients brag about laser treatments.
Laser therapy is fast, usually less than half the time of older forms of physiotherapies. High powered lasers treatments can be as quick as three minutes. Hmmm. And I can rent the electrical stim and portable ultrasound units for home use. I can save on staffing costs, charge for the laser and provide home therapy.
